Patient / Consent Forms
The following forms are required for NEW patients — may they be residents of Houston or visitors from other neighboring communities — and must be accomplished PRIOR to coming to our clinic for your first appointment. The remaining forms will be required based on the reason for your initial visit; we will inform you of the required paperwork once we determine the reason.
