Patient / Consent Forms
Forms are required for all NEW patients — may they be residents of Houston or visitors from other neighboring communities — and must be accomplished PRIOR to coming to our clinic for your first appointment. Any remaining forms will be required based on the reason for your initial visit; we will inform you of the required paperwork after your complimentary consultation.
For your convenience, please complete the forms online.provided above. If you have any technical difficulties we can have the forms filled out in the office, we just ask you to arrive 15 minutes prior to appointment to get the paperwork completed.
We do have a No Call No Show Policy that if under any circumstance you cannot make your appointment or need to reschedule we ask for a 24 hour notice in advance. If we are not informed of a cancellation or rescheduling the appointment will be canceled and charges may be rendered if you wish to reschedule. We hope you understand this policy in order for us to better serve other patients who are in need of care.
